What are the benefits for my patients?
Brook Remote Care delivers continual health monitoring and personalized health coaching, leading to fewer office visits and better health outcomes.
When registered for Brook Remote Care, your patients will receive one or more cellular- connected medical devices (glucose meter, digital scale, blood pressure monitor, pulse oximeter) at no cost to the patient or your office. Patient readings are securely added to the patient chart in Epic on a weekly basis.
In addition, your patient will receive monthly phone calls from a Nurse Monitor from Brook, during which they will review those readings and discuss updates or changes to the treatment plan. In addition, your patients will have access to our Health Coach team.
