How do my patients become eligible for Brook Remote Care?

For your patients with covered chronic conditions (diabetes, hypertension, obesity), once you have determined the medical necessity, you should refer them to Digital Health. Digital Health will conduct an initial telehealth assessment, and will discuss the benefits, insurance coverage and any possible out of pocket costs with the patient before collecting patient consent and referring them to Brook.
