How do I refer my patients to Brook?

For your patients with covered chronic conditions, you will refer your patients to Digital Health for Brook Remote Care and provide the thresholds and health outcome goals in the order. An initial Telehealth visit will be scheduled by Digital Health with your patient. In that Telehealth visit, Digital Health will discuss the benefits and possible out of pocket costs of the service and will obtain verbal consent from the patient to proceed. Digital Health will then place the order through Epic to Brook.
